Observation

During onsite inspection, Licensor observed a roll of clear plastic trash liner bags under the handwashing sink in a preschool classroom cabinet accessible (cabinet was not equipped with a locking mechanism). (2)(d) During onsite inspection, Licensor observed a two tall wall cabinet storage shelves one in the upstairs licensed bathroom and on in SAC classroom not anchored or secured and posing a risk tipping. (3)(f) During onsite inspection, Licensor observed outlets in the SAC classroom that the electrical outlets are not equipped with a tamper resistant outlet, blank plated, or other approved tamper resistant covers. (5)(a) During onsite inspection, Licensor observed two outlets in the kitchen near the food prep sink and one in the upstairs licensed bathroom near a toilet not equipped with a GFCI tamper resistant outlet.
